I read "Cosima d'ignoti (o d'ignotis) intesa Campagna".
So, maybe Cosima (or some of her ancestors) was a foundling who was popularly known by the family name Campagna.

Yes, Vicaria is one of the districts of Napoli, but I think Vicaria also had other meanings, that now are lost. For instance, the "Gran Corte della Vicaria" was a Court in Naples, he may have been a clerk in it. I also found some references in Google Books to "portiere di Vicaria"...
