You may use the same form to ask the certificate to the parish, then it will be sent to the diocese of Tortona for the legalization.

It should be possible that any of the descendants until you has decided to renounce to the Italian citizenship;
when applying, you should declare that this didn't happen and will be verified by the consulate of the area where the persons lived.

As your ancestors after that/those immigrated to the US, they were US citizen, i don't think the US required to renounce to a possible other citizenship. This should be verified for those who arrived from Italy in the case they worked for the US government.

Cemeteries at present are cared by the civil authority (comune), not by the church, at least here in Northern Italy they are open during the day, exact times are shown at the entrance, but in general there is no personnel.

The apostille is requested by the US offices if you need certain processes, not by the Italian, but it's made by the Prefettura, a superior office to the comune. As to the shipping of the certificate, in general they send by the Poste Italiane as ordinary letter (not registered) without asking payment of the postage (even if some comune ask it)..
