Embassy forms?

Over 25 million Italians have emigrated between 1861 and 1960 with a migration boom between 1871 and 1915 when over 13,5 million emigrants left the country for European and overseas destinations.

Embassy forms?

As my progress towards Italian citizenship inches forward and my folder of documents thickens, I still haven't attempted to retrieve any documents from Italy.... My family came from a VERY small town in Pescara (around 300 people, then and now), and according to family lore there was an earthquake in the 1930's that destroyed most of the town.

Assuming that the records still exist, I was going to send the forms listed on the US Embassy to Italy's website here: http://italy.usembassy.gov/acs/records.asp

Has anyone had luck using these forms? Is there anything else I should include? Various websites suggest including a postal credit, but I'm not sure how much that would cost.
