confused about forms 3 and 4 from consular website

confused about forms 3 and 4 from consular website

Who fills them out and signs them. For form 4 it says declaration of deceased Italian Ascendant. Is this in reference to my forefather that came from Italy? But then it says so and so never renounced Italian citizenship before any Italian authority and lived in ____ since age 18 for so many years. Seems weird. So yeah how do I fill that out.

Form 3 is the same thing but for a living Italian Ascendant. The only people left in this line are me and my father. Who is this form in reference to and who fills it out signs it?

Sorry if it is confusing but I know someone can help. Thanks.

Form 3 is for your father (unless he is also applying). If he is applying, then you omit this form and both fill out the application. You fill out Form 3 only for an ascendent who never naturalized. If he naturalized, you omit this form and present his naturalization papers.
