I hope this works. im trying to get my fathers name that is on his birth cert.(mariano) changed to Michael. Illinois vital records states that if i get a court order stating that my fathers name is in fact Michael and his middle name is Mariano then illinois can ammend his birth cert. If this happens then i am good to go. If not then we have to go the way of name change(Money, longer process). Luckily my father is still alive.
