On December 28 I, Paolo Tirone, sacramental chaplain, baptized a child born today, son of the spouses Placido di Puma and Maria Cutaja, which was given the name of Salvatore Maria. The godparents were Salvatore Salvo Gipunello and his wife Rosalia, spouses from this town.

The date on top of the page is December 20th, 3rd Indiction, 94.
The third indiction was from September 1st, 1694 to August 31, 1695
So the date is December 20th, 1694 and the baptism date of your ancestor is December 28th, 1694.

No. But the 3 initial letters at the end of the act for sure indicate the town. The first letter, the "h", means "huius" = from this [town] [name of the town].
Maybe the town is Racalmuto or another town in the neighbours, because the last names Puma and Cutaja are very common in that zone.
