I have a relative in Sicily whose name begins with Don, he is a first born however his daughter in the town book has D2 before the name is that because the father is Don?

Don was a title, not a name. Donna was the female equivalent. Did you really see "D2" or something such as "D." or "Dª"? Don and Donna were often abbreviated.
Sure looks like a D with a 2 off to the top right. Like D "squared".
