Is this a real italian name or has it been changed? From what?

Mongiat is a family name. I researched the name and found that my relatives passed through England to the US early in the 20th century.

Is Monjack a bastardization of the surname Mongiat?

Does anyone know what info is included on a Italian certificate during the 1950's on or in general?

Are parents names included? Deceased spouses? Having trouble getting cert.

I made a request for death cert. This is what I got back. Asking alot with this.
