The act (#1 Cipriano Angiola) was written on October 27th, 1909, upon receiving a note from the Commune of Guardia Lombardi, with a copy of the original act, which follows.
Act date: October 24th, 1909 at 10:15 am
Declarant (father): Cipriano Tommaso, 27 years, peasant, residing in Frigento
Mother: Di Matteo Mariantonia, peasant, residing with him
Birth date: October 20th, 1909 at 8:15 pm

Following there's an annotation written to correct an error where marked with an "a"; they had written the name of the Registrar of Guardia instead of that of Frigento.
The annotation on the left is the "closing" of the register at end of year.
