The document certifies that the foundling Luigi Quarzo born Nov 1 1850 was given to Rosa Gardella (maiden name) Bartolomeo (married name) to be raised.
Rosa belonged to the Parish of Neirone in the town of Cicagna, Genova province. The baby was baptized in the Church of the hospital. Released on 31 Jan 1871.

It is not a formal adoption and does not imply that Luigi changed his surname.
