blank forms for birth,marriage,& death

are there any online resources that one would be able to access blank forms of birth,marriage,and death such as used by the civil records offices in italy? many of the films that i've made reader copies of are very hard to read. this would allow me to record the necessary extracted information onto the blank charts. basically blank standard government forms are what i'm looking for.
