Question about documents

Hi all

I am in the process of gathering all the necessary documents to apply for dual citizenship in NY, however I was reading the consulate website and it said:

"All certificates must be in "long form" or “extended formâ€

The NYC consulate doesn't like or appreciate surprises, so you should definitely ask permission in advance for your father to come along. I presume you mean that he has decided to also file his own application for citizenship?

As to the marriage certificate, no, church-issued documents are not acceptable. You must obtain a state-issued, long-form, document from New York's Vital Records department. Start here: http://www.health.state.ny.us/vital_records/

Thanks for the clarification Johnny - I figured I would have to obtain the state issued documents.

As for the appointment, I think I rushed the conversation because of the 2.49 a minute charge I am fine going by myself - but since I am obtaining citizenship through GGF-->GM-->F I figured he would have to be there? -- I read on the message board somewhere that he would have to apply first before I can? Is that true?

At the bottom of this NYC consulate web page, there are four forms which can be downloaded; if you haven't done so already please grab all of them.

Form 3, Declaration of Living Italian Ascendant, will need to be filled out by your father. You then have two choices, 1) have your father sign the form in front of a notary public who then must notarize the signature or, 2) your father comes to the consulate with you, making certain he has adequate identification with him (driver's license and US passport preferred) and then he signs the form in the presence of the consulate officer.

You will also need to complete a Form 4, Declaration of Deceased Italian Ascendant, for your deceased grandfather. This one will be signed by you (at the consulate), attesting that you have completed the form to the best of your knowledge.
