You are correct it is a birth certificate.
Carmela Bonfiglio was born on Feb 28 1915 to Paolo and Concetta Manciagli
The original birth act is registered in the 1915 birth register of Palazzolo with record #125
The certificate was released on Feb 24 1920 for emigration purposes.
