help translating birth record from 1811

this is a birth record from 1811,

I was able to get most of it, I can't quiet get the birth date and the name of the mother,

any corrections please let me know. This is what I was able to get so far.

Gianmaria son of the deceased Giovanni Mosca of 40 yrs old.
Daughter Catarina born September??? 11???

not sure of mother's name. is number 66 top.

Date of birth is December 1 1811
