Translation help requesting a certificate

I have also posted this on the Naturalization board.

My brother has made an appointment at the Philadelphia consulate in 2019 to apply for his Italian citizenship.

A number of years ago, I wrote to our grandfather's comune and requested and received an uncertified photo copy of his atto di nascita which served my genealogical purposes. I know that my brother is going to need a certified copy of our grandfather's birth act and I would greatly appreciate help composing a letter into Italian. Here's what I believe the letter needs to say:

"Gentlemen/Dear Sir:
I will be applying for Italian citizenship in the United States and am writing to request a certified copy of my grandfather's birth certificate (question: should that be an estratto?). My grandfather was Giuseppe Epifano who was born on 31 August 1889 in the comune of San Mango d"Aquino.

I have enclosed a self-addressed return envelope for your convenience and a copy of my driver's license for identification. I greatly appreciate your assistance in this matter and hope to hear from you soon.

Sincerely,"
