Birth Record translation

On the record # 188 for Girolamo Puma, it appears that there are three names given for the parents - first the mother, than the father, and then a deceased person after the father's (husband's)name. Please review and explain:

Also, is the side note a marriage?

Thank you. Surprised that they had one of the grandparents listed on the birth record.
