Full Translation Baptism Record

Can someone try to translate this for me. I am trying to identify the full text. This is the baptism record for Antonio Caputo. His grandfather Antonio Caputo presented it in Missanello for his grandson. Antonio Caputo was born to Vinvicenzo Caputo and Lucia Izzo. I am trying to identify the rest of the text.

Any updates on this. I am able to read much of it but having a hard time with some of it

L'anno 1909, il 15 di giugno, ad ore 10 e minuti 30, nella Casa Comunale.
Avanti di me, Avvocato Rocco D Petrocelli (?), Segretario delegato con atto del Sindaco in data 3 maggio 1905, debitamente approvato Ufficiale dello Stato Civile del Comune di Missanello, è comparso Antonio Caputo, figlio di Maddalena e padre ignoto, il quale mi ha presentato una fede di battesimo, e mi ha chiesto di trascriverla esattamente e per intiero nel registro di nascita.
Io aderendo alla legittima richiesta di Caputo Antonio, che ha interesse alla trascrizione, ho per intiero ed esattamente trascritto la fede suddetta, che è del tenore seguente:
Chiesa Italiana di santa Maria del Carmelo, Utica, NY, Stati Uniti d'America. Fede di battesimo.
Il sottoscritto dichiara che Antonio Caputo, nato il 11 dicembre 1902 da Vincenzo Caputo e Lucia Izzo fu battezzato dal Rev(erendo) A. Castelli il 12 dicembre 1902. Padrino Nicola Balzano. Madrina Rosaria Barella. Utica, li 1 maggio 1904. Il Parroco Reverendo Joseph Formia.

What follows what I transcribed has no relation to the baptism; it's just the sequence of annotation in the official baptism translation and the usual sentences about witnesses in the Town Hall, and so on.

Thank you so much! The witnesses in the townhall may be relevant. What I am trying to find is if my G.G Grandfather came back to the US after this. I have another thread on this so I wont go into any details here but basically Vincenzo Caputo and Lucia Izzo had a son Antonio. Lucia died in Utica NY in 1906 and after that he was raised by my G.G Grandfather Antonio and his wife Filomena (Antonio's Grandparents). I located a passport in 1908 of them all leaving NY to Missanello and then someone amazingly found this record. It was my G.G Grandfather presenting his grandson. What I dont know is what happened to Antonio (son). I assume he stayed in Missanello and was raised by family there. Either Filomena, Antonio or Lucia's family. Also I dont know what happened to Antonio and Filomena. I dont find them back in Utica and that was their home the entire time he was here. No death, etc.
