Help writing a letter to a commune to request a birth certificate

I am asking someone with experience writing a letter to a commune, what you need (copy of your drivers license?) and a donation how much of a donation to send with the letter and also the address of where I write it to. It’s for Sebastiano Di Maria born 09-18-1894 to parents Mariagiuseppa Cerrone and Michele Di Maria. He would be born in Santa Croce del Sannio, Benevento Naples Italy.
