In a birth record for my "step-great-granduncle" (my great grandfather's half-sibling), it states the mother as "Turci Maria Luigia di Francesco" (I'm blood related to her).
How should I view the middle name? Why do a lot of Italian records have "two first names" or include a middle name? Could it reference her mother possibly?

Maria Luigia without a comma between the two names is actually just one name. It may reference to mother, but it may also reference the father.
