Accuracy of letter requesting a birth act

I have attached a letter that I composed using a template but with modifications. Would someone read it and let me know if it contains any errors that would result in delays. This is my first time writing to officials in Italy and would like to be sure that this letter contains all the information they need. More importantly, that it makes sense to them.
