Possibly the uncle's surname is supposed to be Saltalamacchia (a somewhat common name in modern Italy)?

It would be most interesting to know exactly when (and why) the cognome spelling changed from Bevilacqua to Bivalacqua.

Across the board for all family members? Odd...

Visit this site, click on Advanced Search, and enter bivalacqua in the Groom's last name box. The results (below) appear to be the marriages of your grandfather (bottom) and two of his children - Frank and Salvador Jr.

I also found three death records. Curiously, I was not able to find any birth records at all, using either spelling. Perhaps they were all born in Italy; perhaps they were born in the US but somewhere outside of Orleans parish. I would suggest obtaining one or more of the death records (be sure to get "long form" documents, not simple extracts) as they will contain useful information.

PS: Despite what the web page says, I could not make either the Basic or Advanced search work in Internet Explorer 8. I had to open the page using Firefox instead.

I also found the same thing. would the death records have a place of birth on it as well?

They typically do. However the information is as reported at the time of death, usually by a surviving spouse or adult child. It may be as simple as "Italy" and may be flat-out wrong.

The death certificate will usually include the deceased's social security number. Using that, you can request a copy of the original social security application which incudes place of birth and is generally more accurate since it was completed by the applicant him/herself. See this US government website.

There is some data online; the province of Consenza, for example, has a great deal of searchable vital records for the period of 1800-1900 available online. However this is not the rule - I expect that for every one vital record available online throughout Italy there are 100 (1000?) that are not online.

With the names of one or two likely comuni in hand, you will have some good options for locating vital records. But without them, well it's darned near impossible.

Based on census ages, approximate birth year of 1888 - 1889, I do believe
this could be a good possibility for the WW1 draft registration. Listed as married with four children, birth town of Palermo:

So, April, here's what you can do: visit this site or this one and use the form letter generator to create a letter in Italiano to request Salvador's birth record from Comune di Palermo. I would suggest you try emailing the request to serviziostatocivile@comune.palermo.it , an address I found on this page at Palermo's website (scroll about halfway down the page to the section titled Ufficio Stato Civile).
