Need help for translation of a military record for GF

Just received the military record of my GF Giuseppe Crispino. I can figure out some of it such as height, etc. but could use some help on the rest, especially the description of the results of his conscription.

Also, can you read any of the typewritten notes? I tried to darken the printing up, but that is the best I could do.

Yes, I know that some of it is missing, but I darkened it up as much as I could. The copy I received from Italy was very light. Is there any of it that you can make out?

Minor details:

"Colorful" should be "Complexion"

"Form" refers to the hair type/style. Straight as opposed to curly or wavy, for example.

Here's what I get from the record:

"Private 1st class soldier, class of 1876, temporary discharge granted"
"Called at arms for mobilisation - Royal Decree of may 22nd, 1915" (two days prior the entry of Italy in WWI on the side of the Allies)
"Denounced to the Military Court of Naples as deserter for not showing up at his mobilisation unit"
"Granted amnesty from prosecution with declaration of the Military Court of Naples on June 10th, 1926"
"Enlisted as mobilized reserve according to Law no 1144 of June 27, 1939" (there we go again...)

Does this mean that Giuseppe actually was in the Italian army?

He came to the US in 1910, and registered for the draft for WWI. I have a copy of that document.
