Translation Check

Could someone check to see that I have put the correct information in the correct places on a divorce registration to be sent to the Comune of Laurenzana? I have tried uploading my pdf and the site is telling me that the file is invalid. Any idea of how to do this so I can have my work reviewed quickly?

I swear I'm not as computer illiterate as I come across on here...I tried with photobucket as postimage is blocked at my job, and of course it's cutting some of my info off. I'm gonna try another way.

They are Ok, I would do a little change to the sentence in page 2, after the word "CHIEDE"

"alla S.V. di trascrivere in Italia la sentenza di divorzio emessa dalla"

These typed docs were given to us by the NYC consulate for my uncle to fill out so he can register his divorce in the comune. I only filled in the written part. Does my interpretation of what I wrote fit in with what should be there? His first marriage took place on July 27, 1967 and the divorce occurred on April 30, 1996.
