Form 4 - NYC

My appointment in NYC is this week, and I think I have everything done, except possibly one thing. I am applying GGF - GM - F, so I am applying under category 5. It asks me to fill out form 3 or 4 (3 if living, 4 if deceased) for my GM and F, which I did. But then I realized it never asked for me to fill it out for my GGF.

As my GGF is deceased, it would be form 4. But...

1. It's not on the list of requirements
2. He did naturalize (after my GM was born), and form 4 is asking me to declare that he never renounced his Italian citizenship, which he did

So...do I need to fill one out for him? Anyone else go through this consulate come across this?

Yes, you need to fill it out, although you can do it at the consulate in front of the officer if you need to. The form asks you to declare that he never renounced his Italian citizenship before any Italian authority, which he presumably did not, even if he naturalized as a U.S. citizen (i.e., renouncing it in front of an American authority).
