Need Attorney - Need Mom's NYC Birth Certificate

Hi - I live in CA and am desperately trying to get a copy of my mom's BC to apply for Dual Citizenship. My mom is in assisted living in MA and unable to travel. I've had her mail in requests and had them rejected (twice) because the name and date of birth on her drivers license differ from her BC from 1926 (Rena vs Rafaella). I have a court order in hand that demonstrates they are one-in-the-same. I just don't have time to mess around. Need someone who can help me get this done. Heck - I'll even fly to NYC myself but want to make sure I have the right POA and supporting docs. Don't want to fly all the way there to have to turn around. Need someone who knows their stuff.

Any advice?

Hi Vito. I had written a response earlier, but it disappeared. I will try again. I am pressed for time at the moment, so I can't go into too much detail.

Make sure you read the instructions carefully

It might also be helpful to send in a birth certificate application. http://www1.nyc.gov/assets/doh/download ... birth1.pdf
State that you need the document for dual citizenship in Box 10. You will also need a Letter of Exemplification (check yes in Box 11). This is an additional $5.

Write a letter explaining your situation.
