Translation of Matricular Role

HI there:
I have the matriculate role from Ascoli Piceno on three of my relatives. Can someone help translate this? There is no link because they send me photos of the pages from the government offices. Ascoli Piceno has very little on Antenati. So this has been a challenge.

Rosy complexion
Healthy teeth
Exempted for a serious deformation of the spine

on May 30th 1916 the exemption was confirmed
on Sep 26th, 1917 he was declared reluctant (Italy was at war, then)
on Oct 21st, 1917 he was ultimately exempted for legally proven idioticism, after showing a medical certificate
