Hi Everyone!!

If someone could help.. I Have some questions about getting the SS-5 application data for my late relatives.

Can some one tell me the address of where to write to?
and what I need to give Social Sec. to get the SS-5 information?

If one is not sure of the SS # can you use other ways to describe who you want the # and information for?

And should I also mention "one.. of the freedom of informaiton acts"?
to save time?
I know they want $30.00 and I want to get it right the first time out if I can.

I think I may need this for record keeping and it may be needed for the consulate.
It also may have some correct name spelling on it!..
I plan on getting these for our surnames: DeMeo, Esposito, Forese and Paonnoe
I have not been able to find the info on the site search here yet!

Dear Rich,

The SSN is often (though not always) on the death certificate. In my GF's case, he died in 1958 and is not in the SSDI. But he did have an SSN--which I did not know he had--until I saw his death certificate.

You can request SS-5 online. I'm not sure if the cheaper numident will serve your purpose or if you need the more expensive photocopy of original application to make sure you get all the information.
