One and Same Affadavit

Hi, I was wondering if anyone has had any experience with this Document. I waited 8 months for the Consulate in Philadelphia and now I need to try to put a document together. What do I write? Do I hire a lawyer? What city should I file it in?

Thank you anyone that can provide some help.
