Birth Certificate Translation (Deleo)

Is someone able to help me translate two birth certificates? It would really help me add some valuable information to my family tree!

One is for Francesco De Leo (#821- born Nov 27 1892). There is also a notation on his record, is someone able to translate that as well?

The other is for his sister, Teresa De Leo (born Aug 1888 or 1890). For some reason I found two birth certificates, with the same name and same parents?

son of Gaetano De Leo aged 50 farmer and his wife Angela Amorosini (daughter of deceased Michele)
The side note states that on June 3 1913 the court decided that the record should bve corrected changing the name of the father from Quirino Cataldi to De Leo Gaetano ( but the record dos not mention any Quirino Cataldo...)
