Translation of a record please?

Hi! This is a record I received and would someone mind tranlating for me? I think it is the birth of my Great Grandfather. Thank you!

This is the birth act for Giuseppe dello Iacovo on 11 Aug 1895 in San Leucio del Sannio to parents Lorenzo dello Iacovo and Raffaela Izzo, living at Via Zolli.
Apricena, Caltanissetta, Grottolella, Mazzarino, Montefredane, Salerno, San Severo, Vasto

This is not actually the birth act, but an extract (a hand written copy) of the birth act, issued 27 years later.

Comune of San Leucio del Sannio, Benevento
Extracted from the register of birth acts for the year 1905, number 61, Giuseppe dello Iacovo

August 15, 1905, Maria Furno, age 70, midwife, appeared and declared that at 11:20 a.m. on August 11, 1905 in the residence on via Zolli a male child was born of Raffaella Izzo, wife of Lorenzo dello Iacovo to whom was given the name of Giuseppe.

The midwife reported the birth instead of the husband in order to document her services provided to the said Izzo in the birth and because the husband is in America.
The present document conforms to the original and is issued upon the request of Lorenzo dello Iacovo on March 21, 1932.

Although in the document it looks like the name is written as dello Jacovo, I believe that the name is probably dello Iacovo, since in Italian the letter "J" normally doesn't appear. Throughout the document, however, the letter looks more like a "J" and it is clearly written differently than the letter "I" in Izzo. I notice that on current city phone lists the name appears as dello Iacovo.
