Translation of Birth Record - Epifanio Lo Re

Hello, Hope everyone has been doing well. Can someone help with the translation of this birth record from Melilli,Sicily? I have attempted to translate the document myself there are parts I'm not sure are correct. It is numero 8 on the page

My translation below:

IIn the year 1904, on the sixth day of January at 11:30 a.m., at the Town Hall.

Before me, Luigi Tranchina, fourth deputy to the Mayor and the other deputies, not present

Civil Registrar of the Municipality of Melilli, and appearing before me, Bibisi Rosa, aged thirty-three, a notary?, residing in Melilli, who declared to me that at two o’clock in the afternoon on the fifth day of this month, a woman who does not wish to be named

A male child was born, whom she presents to me and to whom she gives the name Epifanio and the surname Lo Re

Witnesses to the foregoing and to this act were Santostefano Gaetano, aged sixty-four, a laborer, and Abramo Francesco, aged sixty-three, a laborer, both residents of this municipality. The declarant has formally requested to be granted custody of the said child, promising in my presence to provide for his support and care and to hand him over to the authorities upon their request, and finding nothing contrary to this request, I have agreed and have entrusted the child to the declarant

I, the undersigned, have witnessed this document in the presence of the parties involved, and I hereby attest that the declarant is illiterate.
