Help w/ checking a couple lines of translation from BC

I think I've got most of my daughter's BC translated correctly but I'm having some trouble on a couple lines.

Time of birth --- Il Tempo di Nascita
Place of birth --- Luogo di Nascita (or should that be Posto di Nascita)
State File Number --- N. dell'archiviazione di Stato

And then the one's that really threw me:

Mother/Parent's Name before first marriage: Madre/Nome del Genitore prima del primo matrimonio

Father/Parent's Name: Padre/Nome del Genitore

Could you please explain to me why "Parent's Name" would be translated "Cognome"? I'm not sure I understand.

I think the birth cert (which is new) is trying to be "politically correct" and not assume that the biological mother and father will be listed on the record. In this case, the biological parents are the ones named, but I'm not sure how "cognome" expresses what the record is trying to do (if that even matters).

Likewise, the "name before first marriage" is the "politically correct" version of "maiden name," would that difference have to be expressed or would "da nubile" be better because Italian b.c.'s aren't "politically correct"?

This translation will be submitted to the local consulate, so I'd like to be sure it is "literal" enough to pass.

Thanks again for the help. I've never seen the "Parent's name" or the switch from "maiden name" to "name before first marriage" before.
