Husband and wife are 3rd cousins to each other on the same surname line from the same town in Sicily.
Surname males on the husband's side have a disproportionately high incidence of a particular disease - "it runs in the family" - but not on the wife's side.
In the absence of DNA testing, can any hypotheses be drawn about who it comes from?

My guess is the carrier (of the gene tied to the disease in males) is a female spouse on the husband's side. But that's merely a guess...

As I think of it, three examples come to mind - all born in the 1920s or 1930s. They were cousins to each other, sharing the same set of surname grandparents. Their grandmother might be the common denominator. Her female ancestor also married into the family.
