My Grandfather born in Gasperina, Cantanzaro was one of six Sons, birth records show three Sons were born with a middle name and three Sons were born without a middle name. Would that be a specific naming custom or just a coincidence?

there's no naming custom, as far as I know.
However, in Italy we don't have "middle" names.
We may have "double" names, like "Maria Teresa" or "Francesco Saverio", or multiple names, separated by a comma in official records. The first of such multiple names is usually (but not always) the one used in everyday life.
For instance, my name is Giuseppe, Italo, Maria.
