how do i interpret the indices

how does one read the indice as the columns have no explanation. i am specifically looking at Lombardo vito with the notation 12/200/1693 what does that indicate.

also where is this telling me to go
Bonanno Bernardus- Giovanni e Angela
and
Thomasa Russo- Giovanni e Eleanora

is this telling me it is their parents what does 1038 indicate as well as 1736
