Mike Ross changed his name early, and therefore all his kids took the last name Ross.

I ordered my gggrandfather's application, and it had his mother's name on it. It took maybe two weeks to arrive.
The other thing you could try would be ancestry.com to see if maybe someone else happened to upload a copy of it to their family tree as a source. Not likely, but worth a shot if you are ...
