I had to correct my GGF's death cert in PA. A letter to the Funeral Home and then a form with their update sent to the state for correction. Check with the NYC Health Dept to find out how to correct a death cert in the boroughs. If it is relatively easy you may want to just do it.

I have an almost identical situation: US born great-grandmother marries italian, but no record of her 1906 birth. I brought this into the consulate in Chicago and they accepted it as long as she was not on the 'italian side'.

Thanks for the wondeful clarification! I am not willing to wait for my mother to apply in Miami with a year wait. I made my appointment in Chicago with a five month wait.

I plan to bring an extra copy of all the documents to my appointment and ask them if they can let me keep the originals or ...

Sorry, I should clarify. The requirements state,

If your marriage certificate lacks information such as dates and places of birth, you must also obtain a certified copy of the marriage application/license
