I called them first and explained (I live in italy, so it makes shipping and also communicating a tad easier)....they asked me to fax a signed request (typed and printed what i explained on the phone then signed ...

update: did an archive search in the US and found out that my great grandfather naturalised my grandfather when they came to the US (my grandfather was 3 years old, his father included him as family on his naturalisation certificate)....so, at that time, he lost his citizenship by way of his father ...
