If you need a certified copy you must write directly to the Comune of Potenza
NB - I believe there is an error on the record. Father's name is listed as Rocco, but at the bottom of the record the signature of the father is Pasquale Racana

Appearing before the official was Rocco Racana, age 25, a carpenter residing in Potenza, to declare that at 10:30 AM on the 5th of the current month at the place at Strada del Popolo #54, to Ersilia D'Andria, his wife, housewife residing with him, was born a male child who he presented to the official and to whom was given the name ROCCO.
Witnesses to the record: Gerardo Messina, 24, and Giuseppe Ferrara, 25
Signed by the father and both witnesses as well as the official...
