Understanding spouse name

I have a couple of questions about these records. Nuntia was previously married to Jospeph Furnari and on that record she is listed as Nuntia A Spiritu Sanctu. When she marries Pietro Di Marco here, here name is listed as Nuntia Ferrazzano, widow of Furnari ( on the publication of marriage and the marriage record linked below). What would be an explanation for the cognome Ferrazzano as opposed to A Spiritu Sanctu. Also what I cannot make out he hospital name she is from. Can you help, please? Thank you John
