Help with Translating

I need some help translating my grandfather Giovanni Stefano Natale Moschino birth record. He was born December 25,1903 in Ozzano Monferrato. I especially wondered if they listed the address of the house he was born in.

If you could provide a copy of the document or a link to it, that would make it much easier.
Researching Gibellina, Sicily surnames Bivona, Bonafede, Zummo, Ponzio, Bevinetto, Beninati, Fontana, Cipolla, Bruno, Manfrè, Lanfranca, and Navarra
