Translation Request: Birth Certificate Extract

Hi all - I'm hoping someone might please be able to help translate the hand written Italian of this document?

I'm getting some of it, but most interested in the long stretches of hand written Italian. I'm trying to confirm the ages / residences of the baby's parents, the names and ages of witnesses, and whatever the miscellaneous notes are at the very end. Thank you in advance to anyone willing to take a look - much appreciated!

Witnesses: Salvatore Buonocore, 46 years old, tenant farmer; Mario Cinque, 40 years old, laborer.
Only the declarant could sign the record, as the others declared to be illiterate

I'd say 5 Via Massaquano.

And thank you Angela for the street name. I had been looking over a map for what it could be and missed Via Massaquano.

I had thought it was number 10, though, not 5. Am I mistaken in reading that as "Via Massaquano al numero dieci"?

You are right, it's "dieci"!
"Cinque" are the minutes of time of birth.
