According to a 1918 document signed by my Ancestor in Philadelphia, Pa his birthday was Jan, 15, 1897, but his birth record from Gasperina, Cantanzaro says he was registered Feb 23, 1897, that's 39 days after he was born, does that sound right or is one of the documents in error?

Here is the birth act (#23) from Gasperina that is dated 23 February 1897.
The birth of this child was on 21 February 1897.
His name is Innocenzo Giovanni Carchidi, son of Saverio, a barber; and of Mariangiola Siciliano, spinner, his wife.

well I learned something today, thanks so much for the information.
So the registration and birth dates are very close together and my US document is in error, I thought that would be the outcome.

It was generally the case that a child was presented (or delcared if not able to be presented) within 3-5 days to have the birth recorded.
There are other instances, however, when births were recorded after a considerable time. For instance, many immigrant families wanted the births of any child born in USA to be recorded in the ancestral town. Naturally, it was somewhat delayed. Also, a child who was born "parenti ignoti" and who was later recognized by his parent(s) would be recorded in the civil birth register at that time to record his birth, often changing his/her surname. This would be annotated on the original record as well.

My ancestor was born on July 8 and his birth was recorded on July 12 in Italy. His earliest documents in the USA listed the DOB as July 8 but later documents listed July 12. My mother used to always say that he couldn't remember if he was born on July 8 or 12.
Do you have any thoughts as to why there would have been the confusion?
