Francesco Guarino leaving Naples on June 11, 1905 and arriving in New York on July 3, 1905 aboard the S.S. Piemonte (line 15).
What is the address he’s joining?
Was is the heading of the column before condition of health?

Not much of an address here! I read "brother Michele NY Forth St" and would interpret this as someplace in NYC. There is a Forsyth Street in lower Manhattan, a plausible destination. It cannot be ruled out that this is a misspelling of "Fourth", which in Manhattan could have either an East or West designation (though these are often omitted from such manifest listings), and both would also be plausible immigrant destinations. But this scribe elsewhere (small sample size, you can check for more) does not seem to be doing such spelling-out of the numbered streets or avenues. Whichever street it is (and my guess is Forsyth) they all have some length, so without a number, this cannot be pinpointed at all.
The column, more legible in the link here, basically asks whether the emigrant had made a prior arrangement for employment in the USA; my understanding is that such contracts, etc. were not permitted, and would be grounds for denial of entry of the hopeful immigrant.
