Need help with translating Death Certificate

Hello,
I need help with translating portions of the attached Death Certificate for Luisa Mazzoli. She was my grandfather's little sister. I have everything except the names of the witnesses and the phrases after their names near the bottom of the "act". Plus, I can't read the handwriting at the bottom, other than someone is illiterate. Luisa Mazzoli died today Nov 13, 1888. She was 1 year and 7 months. She was a resident of Quercia who had been born there and the daughter of Adamo Mazzoli who was lining in Quercia and Santa Marcinelli (?)who was also living there. Declaring her death at the town hall were Adamo Mazzoli, age 30, and Egidio Mazzoli, age50. Both were illiterate and did not sign the document.
