Indeed! I was thrown by the "born in Italy" language on second line of the form, since you don't need to be born in Italy to be an Italian citizen, as you point out.

Quick follow-up question: Will we also need a declaration for the ancestor we're using to claim citizenship - my mother's grandfather? Or is all the documentation on naturalization (or in our case, lack thereof) sufficient? The language in the Newark consulate's website didn't include him, just specified we'd need one for US-born people in the line - my mom's dad and her.

My lineage was Italy-born grandfather -> US-born father -> myself.

I had to complete a Form 4 for both my father and my grandfather.
