I looked through all the death record indexes through 1910 though with no trace of her
Take Carmine's advice and do a record-by-record search. I think all of us have seen a record omitted from its yearly index.

I found a sister of her son's marriage record dated 1869 and it lists both parents (Rosa Giordano and Francesco Cafarella) as being alive.

Here's what I know so far:
Rosa is listed as being alive in 1868, 1869 and listed as being dead in 1909
Francesco is listed as being dead in 1869, alive in 1869 and then dead in 1909
Their death records aren't in any index but I've started looking through the actual records but a 40 year span is a lot to cover. I tried looking for children and found 3 so far.

I think Carmine is right - an error in the index really is the most likely reason for this problem.

I have not used the italian index but for the English/Welsh index I have found a number of people who have apparently never died or have died without ever being born!

People can be missed off entirely or be so badly misread/spelt that you would never be able to work out that an entry was your ancestor (eg Samuele - recorded as Iovani).

At least with the Italian records you can look at the originals so you still have a chance of finding her death. :-)
