Help Translating Rotunno Birth Record

Hello,

I would greatly appreciate help translating the Rotunno Birth Record # 139 on the attached pages. It is from Vietri Di Potenza, and the cursive characters are hard for me to decipher. I believe it is recorded on two separate pages of the ledger. Images attached.

1868 Nov 19th in the town of Vietri Di Potenza at 10:00 AM. In my presence of the civil records official of Vietri Di Potenza appeared Francesco Rotunno age 37 bricklayer (son of Raffaele Rotunno) and he declare to me, that at 9:00 PM on June 17th in the same year as above in the house on strada Orologio,that from his wife Maria Giuseppa Pucciarelli age 30 (daughter of deceased Michele Pucciarelli) that he declare to me, that a baby is born of female sex that is presented to us and that is declared to give to this baby the name of Francesca Rotunno (spelled wrong in record Francesco) . The presentation and the declaration of the above mentioned is made in the presence of Carmine Grande age 34 (son of deceased Rocco Grande) and Alfonso Carleo age 47 farmer (son of deceased Angelantonio Carleo)

Is there a sample version in ENGLISH for an Italian Birth Certificate? I have an English language death certificate and would also like a sample translated birth certificate? Can you assist and or advise as to where I can find one on line. Thank you
