Please Help us read this 1828 Death Act 112 Paolo Puccio - Francesco & Gaetano Maggio

[molan]

Hi, please help us read a Birth record from Geraci Siculo.

It is the 1828 Death Act 112 Paolo Puccio - Francesco & Gaetano Maggio. I also can't read the notation on the left side of the document.

I've attached a copy of the record.
Thanks
Gary
