Salvatore Di Cuffa's name change

Salvatore has his surname changed to Di Cuffa.
I don't believe or see if it mentions who his father may be.
May I have clarification of this?

Hi James - there are blanks where the father's name should be, and Salvatore was apparently given his mother's surname, Cardillo. The situation is the same in the birth index: Antonia Cardillo is listed but there are blanks where the father's name should be:

I don't know if this means anything to you, but the surname di Cuffa is very rare in Italy today; this map shows the current instances of the surname:

Most of the instances of di Cuffa occur in or around the town of Esperia, which appears in the notation in the margin of the birth record you've provided. As you know, this birth certificate is part of the Roccaguglielma records. At the present time, Roccaguglielma seems to be a neighborhood a half mile beyond the southwest edge of Esperia:
